• Must live in Montour, Northumberland, Snyder or Union County

• Must be between the ages of 19-64

• Must have NO form of health insurance, such as Medicaid, GHP, CHIP, ACCESS, Blue Cross/Blue Shield or Veteran Benefits

• Must be a U.S. resident.

• Must bring in proof of household income which includes, salary/wages, disability, pension, Social Security, unemployment, alimony/child support, workers compensation or 1040 federal tax return

• Financial eligibility is determined by a percentage of the Federal Poverty Level

• Must have a photo I.D.
